Financial Policy

You are financially responsible for the medical services you receive at Leg Pain and Vascular Institute. Copayments and deductibles are due at the time of service in accordance with your insurance carrier’s requirements. If you do not have health insurance, or if your insurance will not cover services rendered, you are considered a self-pay patient and payment is due in full at the time of service.

Your insurance policy is a contract between you and your insurance carrier. You are ultimately responsible for payment in full for all medical services provided. It is your responsibility to ensure services are covered benefits and authorised by your insurance carrier prior to treatment.

Appointments and Cancellations

We ask that you provide reasonable notice if you need to cancel or reschedule an appointment. Missed appointments or late cancellations may be subject to a fee in accordance with our current fee schedule. These charges are your personal responsibility and will not be billed to any insurance carrier.

SMS Communications

By providing your phone number and consenting to receive SMS communications, you agree to receive text messages from Leg Pain and Vascular Institute related to appointment reminders, follow-up messages, and billing inquiries. Message frequency may vary. Standard message and data rates may apply.

SMS opt-in or phone numbers provided for the purpose of SMS are not shared with any third parties or affiliate companies for marketing purposes.

Use of Media Policy

Leg Pain and Vascular Institute may use anonymised imagery taken during procedures to educate patients and physicians about minimally invasive treatments. All personally identifiable information is removed before any imagery is used. We will never include a patient’s name, face, or any identifying features in any media used for educational or marketing purposes.

Advance Directive Policy

Because the scope of Leg Pain and Vascular Institute is limited to elective outpatient procedures, it is the policy of this facility that any life-threatening situation that arises will be immediately treated with life-sustaining measures and the emergency medical system will be activated for emergency patient transport to a hospital facility.

If you have an executed advance directive, living will, or power of attorney, please bring a copy to the facility on the day of your treatment to be placed in your medical record.

Patient Rights

You have the right to be treated with dignity, respect, and courtesy. You will not be subjected to abuse, neglect, exploitation, or discrimination of any kind. You have the right to receive information about your illness, the course of treatment, and prospects for recovery in terms you can understand, and to participate actively in decisions regarding your medical care.
